DOCUMENTS REQUIRED FOR TEMPORARY LOSS OF JOB

The following documents are required to facilitate claims for Temporary Loss of Job.

  • Letter conveying termination of appointment/approval of retirement from employer.
  • Letter of application for 25% RSA balance (below 50 years only).
  • Copy of birth Certificate/ Sworn declaration of age.
  • Last three months’ payslip before retirement.
  • Banks statement of account for the purpose of remitting retirement benefits.
  • Completed copy of standard Notice of Retirement form.
  • Passport photograph
