DOCUMENTS REQUIRED FOR TEMPORARY LOSS OF JOB
The following documents are required to facilitate claims for Temporary Loss of Job.
- Letter conveying termination of appointment/approval of retirement from employer.
- Letter of application for 25% RSA balance (below 50 years only).
- Copy of birth Certificate/ Sworn declaration of age.
- Last three months’ payslip before retirement.
- Banks statement of account for the purpose of remitting retirement benefits.
- Completed copy of standard Notice of Retirement form.
Passport photograph
