DOCUMENTS REQUIRED FOR NORMAL RETIREMENT

The following documents are required to facilitate claims for Normal Retirement.

  • Letter of approval from employer granting retirement.
  • Copy of birth Certificate/ Sworn declaration of age.
  • Last three months’ payslip before retirement.
  • Bank statement of account for the purpose of remitting the benefits Completed Standard Notice of Retirement Form.
  • One (1) passport photograph.
  • Complete programmed Withdrawal Agreement slip and Mode of pension withdrawal either monthly or quarterly

OR

  • Application for movement of RSA balance to an Insurance Company for purchase of Annuity plan.
