DOCUMENTS REQUIRED FOR NORMAL RETIREMENT
The following documents are required to facilitate claims for Normal Retirement.
- Letter of approval from employer granting retirement.
- Copy of birth Certificate/ Sworn declaration of age.
- Last three months’ payslip before retirement.
- Bank statement of account for the purpose of remitting the benefits Completed Standard Notice of Retirement Form.
- One (1) passport photograph.
- Complete programmed Withdrawal Agreement slip and Mode of pension withdrawal either monthly or quarterly
OR
- Application for movement of RSA balance to an Insurance Company for purchase of Annuity plan.
